New Look S2S Website Launched

Welcome all to our new look website. We hope you will be able to navigate the site and find information that you require regarding the S2S suite of applications as well as resources available for referral and training requirements.

We are very proud to say that the S2S eReferral and service coordination system as well as the eWaitlist system are now being used by over 5,000 health and community sector workers in Victoria and New South Wales.
In Victoria, we work with 12 Primary Care Partnerships to facilitate secure electronic referral and communication between a diverse group of services as part of the overall ehealth and service coordination framework. 

The systems are used in over 500 agencies/sites comprising about 1,500 services/programs. In NSW there is an eReferral trial in Clarence/Graton region that involved HACC and health services whilst three regions (Mid North Coast, Far North Coast and Southern) are using the system for disability respite coordination work.

In Victoria the eWaitlist system is now being used by the aged care assessment teams in all regions (except in the Loddon Mallee region) to list clients waiting for community aged care packages CACP) as well as EACH and dementia EACH packages in a centralised waiting list which gives access to providers with vacancies to be able access them in a timely manner.

The Infoxchange S2S eReferral system is now integrated with the Royal District Nursing Service (RDNS) client management system Camillus to facilitate eReferral communication between the S2S agencies and the nurses at RDNS in a secure and more efficient process. There is also a trial underway to integrate with GPs using HL7 (via Argus).

It is promising to see that the staff members at agencies that use the system are making use of the valuable service coordination functionality that is available in the S2S system as there are on average 9.7 referrral views per referral and 3.5 status changes per referral. This means that for every referral that is being received the status changes such as Acknowledge -> Action Initiated -> Service Provision -> Closed, are being used to give the service providers valued information required regarding their clients care.
